New Client Paperwork
For new clients or clients not seen in the past 12 months
If you are a new client, or an existing client who has NOT had a treatment at VIDA Aesthetic Medicine in the past twelve months, please submit the required paperwork below at least two business days before your scheduled appointment.
Using Adobe Acrobat Reader on a laptop or desktop computer, complete and save the first two forms below (plus the third if you’re a Botox client), then send as e-mail attachments to info@vidaskincare.com:
