VIDA Aesthetic Medicine is committed to maintaining a safe and healthy clinic environment for our staff and clients. Please be sure to review our updated safety protocols prior to scheduling. New Client Paperwork

If you are a new client, or an existing client who has NOT had a treatment at VIDA Aesthetic Medicine in the past twelve months, please submit the required paperwork below at least two business days before your scheduled appointment. For new clients, we will contact you to schedule your consultation once we have received your paperwork.

Complete both forms below and submit online directly from any internet-connected device:

• COVID-19 Consent Form
• COVID-19 Patient Safety Questionnaire

Using Adobe Acrobat Reader on a laptop or desktop computer, complete and save the first two forms below (plus the third if you’re a Botox client), then send as e-mail attachments to info@vidaskincare.com:

• Client Registration & Medical History (“Submit” button is disabled; use File>Save for e-mailing)
• General Policies Informed Consent
• Neuromodulator Informed Consent (Botox clients only)

It is VIDA Aesthetic Medicine’s policy that clients review, update, and re-sign their medical history and any treatment consents annually. If you haven’t been seen in twelve months, please know we will require a few additional signatures as our new COVID-19 safety protocols require that you complete new paperwork and resubmit electronically. Thank you for understanding.

Existing Client Paperwork

If your last appointment at VIDA Aesthetic Medicine was more than one year ago, please refer to the previous section. If you were last seen within the past twelve months, please complete the following forms and submit them online from your smartphone or desktop computer:

• COVID-19 Patient Safety Questionnaire
• COVID-19 Informed Consent Form*

*If you’ve been treated since 5/26/20, we already have your Informed Consent on file and you do not need to resubmit.
